Can I download the Application Form?
Yes! You can download the Candidate Information Guide (CIG) which will give you all the information you need to prepare for the exam as well as the exam processes. Just be careful as it is 28 pages long! You will also need to download the Exam Application Form and the Referee Form. It can all be downloaded from the Download Centre under RESOURSES on the navigation bar or by Clicking Here.

Can I have a hardcopy of the Application Form?

How do I complete the Education Requirement section?
Your Candidate Information Guide (CIG) has all the information you need to help you complete your application from page 16 to 21. If you need assistance with Question 8- Professional Lactation Education Hours you can read page 19 of the CIG or Click Here.

To read Question 13, answer the questions, sign and date your application before you post it. Applications not signed constitute an incomplete application and a fee applies.
